Terms & Conditions

By engaging in services provided by GeriCare at Home LLC, the client agrees to the terms and conditions outlined in this agreement. These terms may be updated at anytime, and clients will be notified of any material changes.

Services Provided

GeriCare at Home LLC offers non-emergency, in home nursing care under the scope of a Licensed Practical Nurse, which may include:

Medication administration and reminders
Chronic condition monitoring
Wound care
Vital sign checks
Post-operative support
General health and wellness checks
ADL (activities of daily living) assistance
Other services as agreed upon in writing

All services are provided in accordance with applicable state laws and the LPN scope of practice. Services do not include emergency care, medical diagnosis, or treatment beyond what is legally permitted for an LPN.

Scheduling and Cancellations:

Appointments must be scheduled at least 24 hours in advance.
Cancellations with less than a 24 hours' notice or no shows may be charged a $50 cancellation fee or the full visit fee at the providers discretion.
Repeated cancellations or no-shows may result in termination of services.

Payment Terms

Payment is due at time of service unless prior arrangements are made.
Accepted payment methods: Cash, credit card or Zelle.
Invoices for Packages or ongoing care must be paid within 7 days. Late payments may incur a $25 late fee.

Client Responsibilities

Client agree to:

Provide a safe and sanitary environment for in-home care.
Disclose all relevant medical history and medications
Follow care instructions provided by the nurse
Inform the nurse immediately of any changes in health condition or medications
Treat the provider with respect and professionalism.

Medical Disclaimer

GeriCare at Home LLC does not provide emergency medical care. If the client is experiencing a medical emergency, they should call 911 or go to the nearest hospital. The nurse does not diagnosis or prescribe and works within the LPN scope of practice as regulated by state law.

Limitations of Liability

GeriCare at Home LLC and its providers are not liable for any indirect, incidental or consequential damages related to the use or misuse of services. While we strive to deliver high quality care, the client assumes responsibility for following medical advice, reporting issues and maintaining a safe home environment.

Termination of Services

Either party may terminate services with written notice. GeriCare at Home LLC reserves the right to discontinue services due to non-compliance, unsafe environment, harassment or failure to pay.

Governing Law

These Terms and Conditions are governed by laws of the State of Florida. Any disputes shall be resolved under jurisdiction of the courts in Orange County, Florida.
